Create New Survey

 

Use this Kwikwap function to create a new form/survey for your website. This is a great feature to add to your website to gather information from visitors (to update your database info), use as a registration form for events, explore customer requirements, needs and/or feedback, or to use for competition entries. The applications are limited to your imagination!

 

Remember that you can allocate the your forms/surveys to specific pages now. If you don't allocate a form to a specific page, it will display as a link option on your website menu.

 

  1. Enter the survey title, description and thank you yext in the appropriate text boxes, and click on the "NEXT" button.
  2. Click on the "ADD QUESTION" button.
  3. Select which TYPE of response you want to your question (See below).
  4. Click on the "NEXT" button.
  5. Enter the required information, which will vary, depending on what type of response you have selected.
  6. Click on the "NEXT" button.
  7.  You will now be taken to the main "edit survey" window again.
  8. Repeat steps 2 - 6 for each question you want to load onto your survey.
  9. Click on the "FINISH" button.
  10. Click on "Back to surveys/forms list".

You will now be shown a list of all the forms and surveys you have created.


Click on "Allocate your surveys" to link a survey to a particular page.
OR
Click on "Click here to show the survey/form on your website"
OR
Click on "Click here to show in the members section"

Without clicking on one of the three options above, your form / survey will be stored in your Management Portal, but WILL NOT be displayed on your website.
